Who We’re Seeking: The Hospice Patient Care Coordinator is responsible for supporting and coordinating day-to-day clinical operations in collaboration with the Hospice Clinical Manager to ensure timely delivery of patient care and to improve clinical/quality outcomes, patient satisfaction and financial sustainability. The Patient Care Coordinator also functions as a clinical resource and mentor for the Hospice nursing staff, as well as for the entire interdisciplinary team.
Your Responsibilities: As a member of our dedicated team, you’ll:
• Empower clients in exploring care options and alternative plans to achieve desired outcomes.
• Be skilled in conflict resolution, critical thinking, resource utilization, decision making, customer relations, nursing process, and time management.
• Problem-solve day-to-day clinical operational needs from patients, caregivers, and interdisciplinary team.
• Prioritize and manage multiple priorities simultaneously in a fast-paced environment.
• Utilize nursing process adhering to Vermont Nurse Practice Act.
• Work effectively within interdisciplinary team.
• Excellent communication skills (person-to-person, telephone, written, verbal, group facilitation).

Requirements:
• Minimum Education: A BSN or a graduate of an accredited school of nursing required.
• Minimum Work Experience: 3 – 5 years or more of clinical experience in a healthcare setting; preferably Hospice or Home Health.
• Required Licenses/ Certifications/Background Checks: Valid Vermont Driver’s License and possess a current nursing license in Vermont. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background, and Office of Inspector General (OIG) check/review.
